INSURANCE and FEES

I am a fee for service provider. I do not accept any insurance. If you are interested in receiving services and would like to know if your insurance company will cover any of my services, please contact your insurance company to ask if you have out-of- network benefits. If you do, there will be a deductible before they will pay a percentage of my fee.

After each session or once a month, (depending on your preference) I will provide you with a fee slip. The fee slip will include all of the information you will need to file your claim. Claim information is usually found on the back of your insurance card.

My fees are 150.00 per 45 minute session for individuals and 170.00 per 60 minute session for couples.
